Initial Visit

The initial evaluation usually includes a history, a physical examination, a review of medical records and a review of relevant laboratory and X-ray findings. We ask that you bring pertinent records and X-rays provided by the referring physician and a list of your current medications. This first visit may last 1-2 hours. Please complete the patient information and health history forms prior to your arrival. Insurance card and co pay are required at the time of your appointment.

Insurance

We Participate with many HMO/PPO’s, and also accept assignment on Medicare. You will continue to get a monthly statement for your records. Remember that Medicare and health insurance companies usually do not cover the entire cost of your visit and that you are personally responsible for the full amount of these expenditures. Please check with our Patient Advocate and your insurance company to verify your health plan benefits.
